I acknowledge and represent that my participation in the Continental Ski Club ("Club") is completely voluntary. I understand that my participation in the Club may include participation in indoor and outdoor recreational, sports, competition and other activities. I further acknowledge that while Continental Airlines, Inc. may sponsor certain of the Club's activities, the Club is not part of or otherwise affiliated with Continental Airlines, Inc.

I AM AWARE THAT PARTICIPATION IN INDOORS AND OUTDOORS RECREATIONAL, SPORTS,
COMPETITION, AND OTHER ACTIIVITIES HAS A RISK OF INJURY AND MAY BE DANGEROUS. I AM AWARE THAT I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH MY KNOWLEDGE OF THE DANGER INVOLVED AND I HEREBY AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY, DEATH, OR PROPERTY DAMAGE. I VERIFY THIS STATEMENT BY PLACING MY INITIALS BELOW.

I for myself, my heirs, executors, administrators and representatives, do hereby agree to release and hold harmless the Club, Continental Airlines, Inc., cooperating organizations, and any other parties connected in any way with activities sponsored or promoted by the Club, together with their respective officers, directors, employees, agents successor and assigns (the "Sponsors"), singly and collectively from and against any liability for any death, injury, harm, loss, inconvenience, or any other damage of any kind whatsoever (including any death, injury, harm, loss, inconvenience or other damage caused by the negligence or other acts of the Sponsors), which may result from or be connected in any way with my participation in Club activities.

In addition, I hereby represent that I am physically capable of participating in the above mentioned activities, that my skiing equipment and any other equipment I may use to participate in these activities are in working and safe condition, that I will observe all applicable safety rules and that I will generally conduct myself in a safe and prudent manner while participating in these activities, and I hereby absolve and hold harmless the Sponsors from any damage I may sustain because of any breach of these representations.

I hereby consent to and permit emergency treatment in the event of injury or illness while participating in Club activities. I also hereby give permission to the Club and Continental Airlines, Inc. to use my name and any pictures/video taken of me during these events in any promotional materials, publications, or on the Internet.

Notice: Waivers/Releases of participants under the age of 18 must be signed by a parent or legal guardian. Any participant under the age of 18 must also be accompanied by an adult (21 or older).

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS.
I AM AWARE THAT THIS IS A RELEASE OF LIABILITTY AND A CONTRACT BETWEEN THE CONTINENTAL SKI CLUB, AND MYSELF AND SIGN IT OF MY OWN FREE WILL.
